| Developmental Screening | Free Developmental Screening for children birth up to age 3. Held at various locations within Peoria, Tazewell, Woodford, Henry and Stark Counties. Screening will teach families about their child's development and what their child's next steps are. Screening is recommended 3 times during first year of life, 2 times during second year and at least 1 time a year up until the age of 5. These screenings are educational and intended to make sure that every child is found if they have a delay in their development for any reason. For an appointment please call (309) 999-7030. |

| FREE Evaluations and Assessments | Allied Agencies | Early Intervention is the system point of entry for children who have a diagnosed disability, a delay in their development, or are at risk for delays in their development. Early Intervention will help families with scheduling FREE evaluations and assessments. Early Intervention will assign a Service Coordinator to work with the family to determine the types of evaluations needed. Services for Early Intervention include: Physical Therapy, Speech Therapy, Occupational Therapy, Audiological Evaluation, Counseling, Nutrition, Medical Diagnostics, Nursing, Social Emotional Consultation, Service Coordination, Parent Liaison, and Assistive Technology. |
| Parent Liaison | Parent Liaison is available to help families through the Early Intervention program. Parent Liaisons are parents of children who have been involved in the Early Intervention System. Parent Liaisons are available to assist with parent communication, resources and referrals, and as a parent to talk with. Parent Liaisons are available to attend meetings, assist with transitions or any concerns you have as a family. |

| Service Coordination | Child and Family Connections is the system point of entry for Early Intervention Services. Families are assigned a Service Coordinator as they enter the system. Service Coordinators will complete intake with families. After intake Service Coordinators will schedule evaluations based off of the families concern. After evaluations are completed Service Coordinator will schedule your IFSP or Individualized Family Service Plan. Service Coordinators will facilitate meeting and will assist team in writing goals. After goals are written services, additional evaluations, additional referrals are discussed. Service Coordinator will then discuss options with family in regards to family choice of providers and IFSP plan. Service Coordinator will implement plan with parents permission and will make referrals as recommended. Service Coordinator will follow up with family monthly, will review plan at 6 months, and will hold annual IFSP. Service Coordinator will begin discussions of transition to a 3-5 program as child is closer to timeline. Service Coordinator will assist family in transition process. Service Coordinators will make referrals as needed. |

| Transition | Allied Agencies | Early Intervention will assist families enrolled in the Early Intervention system to a 3-5 program. Service Coordinators will hold an individual meeting with families to review "When I'm 3, Where will I be" family workbook, and families are given their rights DVD. Service Coordinators will review all options with families in their community and what options family has. Service Coordinator will refer family to programs of families choice. Service Coordinators will also hold a Transition Planning Conference with family and their local school district to discuss all options they can offer families. Service Coordinator will attend other meetings as requested by families. |
