To further assist you, we have developed a list of information and items you will need when you visit the funeral home. Please have the following with you for your initial meeting with funeral home staff:

  • Deceased's full legal name: first, middle, and last
  • Date of Birth and age
  • Birthplace: city, state, and country
  • Marital status
  • Name of surviving spouse, if applicable
  • Veteran's Discharge Papers, if applicable
  • Social Security Number
  • Occupation and place of employment
  • Education, highest grade completed
  • Residence: number and street, city, county, state, and zip code
  • Father's name: first, middle, and last
  • Mother's name: first, middle, and last, including maiden name
  • Name of surviving relatives and cities in which they live: spouse, parents, children, brothers, sisters, and grandchildren
  • Cemetery name and location, if applicable: city, county, and state
  • Church affiliation
  • Organizations or memberships
  • Special achievements
  • One recent photograph for obituary and cosmetic purposes
  • Clothing, including undergarments, socks, dentures, glasses, and jewelry
  • List of 6 to 8 pallbearers, if applicable
  • Life insurance policies
