| Obtaining Information | Information with regard to obtaining a vital records, marriage licenses, raffle licenses, liquor licenses, registering as a business under an assumed name, notary commissions, tax redemptions, tax calculations or tax extensions should be directed to the Vital Records Section of the office at 309-672-6059. Questions with regard to elections, voter registrations, economic interest statements and new addresses should be directed to the Elections Sections of the office at 309-672-6070. Requests for public records under the Freedom of Information Act may be made in person during normal office hours, by e-mail to  or by mail to Lyn Schmidt, Chief Deputy, 324 Main Street, Room 101, Peoria, IL 61602.
