Peoria County Clerk: Passports
Our passport hours are Monday-Friday, 8:30 am to 4:30 pm and no appointment is necessary. In order to apply for a passport you must satisfy the following basic requirements:
- US Citizenship/Nationality
- Citizenship Documentation
- Previous US Passport
- Certified Birth Certificate
- Naturalization Certificate
- Certificate of Citizenship
- Identification Documentation
- Driver's License (not temporary or learner's permit).
- US Passport (not expired)
- Naturalization/Citizenship Certificate (issued within the last ten years)
- Government Employee Identification Card (Federal, State, or Municipal)
- Official Military Identification Card
- Photographs
- Good likeness, full face, normal street atire
- Two duplicate, 2"x2" overall, with image size no less than 1" and no more than 1 3/8" measured from the top of the head to the bottom of the chin
- Plain or light colored background
- Must be able to withstand 225 degree Fahrenheit
- Fee
- Personal checks, cashier's checks or money orders only
- $75 for adult, 16 years or older (receives 10 year passport)
- $60 for minor, 15 years and younger (receives 5 year passport)
- $25 execution fee retained by the County Clerk's Office
- $60 expedite service (processed within 10 business days at agency)
- $31.10 payment for express mail to passport agency and back to client
- Proof of Departure Date on hand carry
- Authorization form for 3rd party to hand carry and receive completed application

- If you satisfy the following requirements you may download an application for passport renewal. (Please note: Applications for passport renewals are not processed in the County Clerk's Office. You may only renew your passport by mail.)
- You can submit your most recent passport
- You were at least 16 years old when your most recent passport was issued
- You were issued your most recent passport less than 15 years ago
- You use the same name as the more recent passport, OR, you have had your name changed by marriage or court order and can submit proper documentation to reflect the name change
- Personal Appearance
- Both parents must appear and sign in front of a passport acceptance agent for a minor child under the age of 16.
- All first-time applicants and persons applying beyond the period of renewal must appear in person.
- Persons applying for passport renewal may do so only by mail. The County Clerk's Office does not process renewal applications.
