Change of Address

You can easily change or correct your address by completing the form below. If your pet is currently vaccinated and registered and you received a reminder or overdue postcard in error, please include your new tag number in the appropriate field so we can correct the information. If you include your phone number or e-mail address, that information is held confidential in our office but allows us to follow up with you if necessary. Thanks for being a responsible pet owner!
