Recycling and Conservation: Quarterly Report

All businesses, schools, and government agencies must submit a Quarterly Report regarding their recycling efforts. Print, complete, and submit this form by fax (309-681-2558) or mail. Please fill out your complete business/organization name.

Quarterly report due dates are:

• First Quarter (Jan., Feb., March) - April 30
• Second Quarter (April, May June) - July 31
• Third Quarter (July, Aug., Sept.) - October 31
• Fourth Quarter (Oct., Nov., Dec.) - January 31
