Wellness: Discounts
$500 Wellness Benefit for Health Plan Members
Members of the Peoria County Employee Health Plan are eligible for $500 in free wellness screenings each year. Examples of eligible expenses include routine health screenings, annual physicals, and well-baby visits. Once the $500 benefit is exhausted, the remaining expenses are subject to co-pay and deductible. Please contact Sage Technologies, the Third Party Administrator of the health plan, at 687-3502 x6254 to determine if your upcoming screening qualifies for reimbursement.
Fitness Reimbursements
As part of the newly expanded wellness program, reimbursement for Health and Fitness Activities is now being offered to ALL EMPLOYEES, not just those on the health plan. The benefit is half of the single monthly membership cost when the employee attends the health club eight (8) or more times a month and 100% of the single monthly membership cost when the employee attends the health club twelve (12) or more times a month. Please note that this is for employees only, not dependents. If you have a family membership, you will be reimbursed at half of the single rate. If the health club you are interested in attending is not included below, please contact the Patient Advocate at (309) 672-6071 to find out if it qualifies prior to registration. Please call the individual health club for further information regarding registration and monthly membership rates. Initiation and/or registration fees are not reimbursed.
When registering, don't forget to ask about corporate discounts. Many facilities offer corporate discounts once a certain number of individuals register at that particular location, so be sure to mention that you are with Peoria County.
